New Patients:
You will be filling out New Patient Forms with our unique Nextpen that interfaces with our EHR (electronic health records). Therefore, it is imperative that you arrive at least 1/2 hour prior to your scheduled appointment time. Failure to do so may likely result in you having to reschedule your appointment.

We ask that new patients complete the following forms and bring them with you to your first appointment:

  • If your insurance requires a referral it is your responsibility to make sure that this is in place prior to your appointment. Failure to do so may result in us having to reschedule your appointment.
  • To complete the registration process in a timely manner we ask that you arrive 15 minutes prior to your scheduled appointment time.
  • Bring your current insurance card with you for accurate billing purposes.
  • Be prepared to pay your co-pay, if applicable, at your appointment. We accept cash, checks, Visa and MasterCard.

Your first appointment is an extended time slot reserved in your name so that your provider can thoroughly assess your needs. If you fail to show or fail to provide sufficient notice that you need to reschedule we reserve the right to charge your account a $50 No Show Fee. In addition, we reserve the right to refuse to reschedule a new appointment for you in the future. As a courtesy to the physician who referred you to our practice, we will notify them of your failure to show.

 

 

 

Existing Patients:
If any patient information has changed since your last office visit (e.g., insurance information, change of address, employer or pharmacy information), please plan on arriving 15 minutes prior to your scheduled appointment time to inform us of these changes.

  • If you are an existing Infusion patient and have had an insurance change you must notify us prior to your next scheduled Infusion so that we may verify benefits and obtain an authorization, if applicable. Failure to do so may result in us having to reschedule your infusion appointment when you arrive.
  • If you are an existing patient that has an insurance that requires a referral it is your responsibility to make sure that your current referral has not expired prior to your next visit. Failure to do so may result in us having to reschedule your appointment when you arrive.
